@drchriszoumalan: Lower lid retraction can occur from prior surgery, trauma, or just with aging. The lower lid position changes and becomes more rounded and hangs lower than it should, causing symtpoms such as irritation and tearing for instance. In cases where I'm performing a lower lid blepharoplasty and the patient subsequently has lower lid retraction, I will address it at the same time as a blepharoplasty by adding a spacer graft such as Alloderm graft as seen with this patinet of mine. The graft helps elevate the lower lid to a better restorative position. In his case, midface volume loss was also improved with fat grafting to further support the lower lid positon and help the Alloderm graft as a support. ⁣ What’s happening in the OR? A lower lid blepharoplasty surgery with fat repositioning through a transconjunctival approach (non visible incision) A custom-cut sheet of Alloderm (an acellular dermal matrix) is placed like an internal hammock, reinforcing the lid and preventing future pull-down.
