Trigeminal neuralgia involves the malfunctioning of the trigeminal nerve, which is the fifth cranial nerve responsible for transmitting sensory information from the face to the brain. CAUSES 1. Compression of the Trigeminal Nerve: The compression or irritation of the trigeminal nerve by nearby blood vessels As these blood vessels pulsate, they can exert pressure on the nerve, resulting in its dysfunction and the characteristic pain associated with trigeminal neuralgia. 2. Demyelination: the loss of the protective myelin sheath that surrounds the nerve fibers. It can be associated with conditions such as multiple sclerosis (MS), where the immune system attacks the myelin, leading to nerve dysfunction and pain. 3. Nerve Hyperexcitability: The abnormal electrical impulses can occur spontaneously or be triggered by normally innocuous stimuli such as touch, cold air, or chewing causing sudden and severe pain characteristic of trigeminal neuralgia. 4. Structural Abnormalities: Other structural abnormalities, such as tumors or cysts, can also exert pressure on the trigeminal nerve and cause trigeminal neuralgia. PATHOPHYSIOLOGY: 1. Trigeminal Nerve (Cranial Nerve V) It is divided into three branches: a. Ophthalmic Branch (V1): supplies sensory information to the forehead, scalp, upper eyelid, and the bridge of the nose. b. Maxillary Branch (V2): supplies sensory information to the lower eyelid, cheek, upper lip, and upper teeth. c. Mandibular Branch (V3): innervates the lower lip, chin, lower teeth, and the muscles involved in chewing, such as the masseter and temporalis muscles. 2.
