waveform looks good, looks like a bad fan module. i would also check fault codes in other modules.

Try and do a initial start up if the fan control unit or it can be the power control module that took a shit

Don’t forget to clear all codes sometimes if you find a problem, the computer can lock things out

Id use a power probe and externally activate the fan to verify the fan is actually working.
