Happened to us a few times when I was new. One driver left with our forklift and operator in it. The next one almost fell out, but got hung up on the undercarriage when the rear wheels dropped out.

First thing you should do is take the drivers keys. Was mandatory at my work place when I was a FLT driver.

We have wheel chocks and a red/green light system, and we still had a driver pull away from the door with a lift on the dock plate and they still pulled away (twice).
