This is why you check the cowling latches under the engine on the walk around. Assuming it wasn’t latched properly, that’s 2 layers of caution failed, which is extremely rare

Correct. However as a former Boeing mechanic who installed the engines. We do install the TRs and Fan Cowles that go over the engines. But this isn’t a Boeing issue. It’s a southwest maintence issue.

Imagine forgetting to lock your car door, having someone open it and steal your stuff, then blaming the car manufacturer for it. This is 100% a maintenance error.

My best gas is pilot called down for oil in the engine maintenance opened up the outboard Cal put the oil in and forgot to latch it

That was an engine nacelle. Odds are the engine manufacturer doesn’t make the nacelles. For example Rolls Royce/Pratt Whitney makes engines, Spirit makes their nacelles.
