Two things can be true at the same time: they can be in psychosis, and there can be something in their ear. Plus what does it hurt to put them at ease if possible.

Does it ever happen that the psychosis based complaint is also some communication for a real problem? Like I feel like bugs in the ear could turn out to be an ear infection?

I cleaned for a lady with dementia who thought she had mice. I looked really hard and found no signs. it made her feel better. over and over I looked and found nothing, each time she felt better.

If another provider found something in their ear after you told them you wouldn’t check, they’d lose trust in psych.

I had a professor say that hypochondriacs die of something. We have to rule out causes of symptoms. You have to investigate.

i worked in the ER for two years and listen bugs go in ears sometimes. i always said if i come in screaming check there first.

omg I actually had a bug in my ear once and the nurse thought I was in psychosis. nurse rolled her eyes as the doctor looked. I'm still annoyed about the interaction years later

Spent nearly 20 years working with drs to try and psych medicate formication & other sensations skin to find out it was neuropathy and nobody did a single nerve-related test 🫠
