You put your thumb&4th fingers in the hole. The index& middle fingers give support on the body during use!

Wait they don't teach you both grips in the US? I was taught to hold with finger grip for precision and palm grip for more force.

palming the instrument or using thumb and RING finger is correct. you cannot suture in a depth with narrow access by palming the instrument

In oral surgery some of us prefer much smaller needle drivers to get in those tiny mouths 🪡 👄 & I keep a small scissor on my ring finger flipped backward ( for lac repairs in the ED)

We call these Needle holders didnt know they are called needle drivers. Palming is optional here they always teach us to use the holes.
