love it for the hospital but don’t always love when pts are DC’d on it who may benefit from a more simplified regimen 🥹 -pharm student

Now try to explain this in an outpatient pharmacy setting to a patient who has never injected insulin before and has a huge sliding scale plan with different increments all written on their label

I just use whatever medium dosing there is. If they need more than they need medicine on board. -Gen Surg

Why do docs order sliding scale insulin on people who aren't diabetic😅😭

also: AM hyperglycemia is a PM long-acting insulin problem. Increase the basal dose.

This is awesome, one thing I’d add is that on admission, we can lower the total home dose by anywhere from 20-50% bc pts usually eat much less in the hospital, to avoid hypoglycemia. -IM resident

It pains me that we are still stuck on sliding scales in the hospital but i understand how it simplifies dosing

T1D here 🙋🏻♀️; I can’t wait for the day that once admitted the pt gets fitted with a closed loop pump and CGM system for the length of the hospital course.
Do you need multiple days of “discrepencies” to make a change or do you change it daily according to need?

My husband who’s T1D does 1 unit for every 50 glucose levels. 250 = 3 units (to stay around 100. 400 = 6 units

How do you determine whether to use a low sliding scale or a higher sliding scale?

Wonderful explanation! I love teaching this to residents; it makes so much sense and it’s cool to see the instant glucose improvements when you make the right changes!

I have type II patients that are on glargine but no short acting? We check their BS at meal times but no blouses are usually required unless the family visited and brought treats

I'm assuming this is for T2D? As a T1D this seems....dangerously general?
