i think it’s also more nuanced than either “psych facility bad” or “psych facility good.” my inpatient stay was genuinely the worst experience of my life, but it also saved my life.

Would like to hear your perspective on how patients with complex medical needs are treated in psych. facilities. As someone who has relied on TPN & tube feeds, I’ve heard horror stories about…

thank you for being that person at 2 am. when my kid was having a mental health crisis and I was shocked at how casual Dr's were with admitting him inpatient.

Peds psych is all I know but god it’s awful. Especially the behavioral patients who need NBU beds bc of severe autism

I have been an attending psychiatrist for 4 years now and I want to thank you for reminding me how much I can still learn from residents. Your content is refreshing, honest and hopeful.

since public health and psychiatry are so intertwined, how were you able to realize you wanted to make changes on a person-to-person level rather than on a community one? asking as a recent grad (1/2)

Agree 100%. I feel the exact same as someone going into psych. Hope to be an SMI outpatient doctor with the goal of preventing hospitalization and maximizing QOL

%100. as a past child psych patient, i also think the way we handle adolescent psych admissions needs to be at the forefront of this conversation.

I work in social work in my hospital’s in pt psych unit. I feel that the overall goal is only admit if absolutely necessary, only keep as long as absolutely necessary, and do everything to set them up from success upon discharge.

You articulate how I feel about psych, as an ED psych RN + prior working in psych for a decade. Appreciating this video so much!

In my city, it’s often inpatient or jail. Unfortunate, but at the end of the day I think we take good care of our patients. Feed them, make sure they get their meds, and have a warm bed.

In your opinion, is there a country that does it better than the US? I’m curious to understand what they do differently and how it may be possible to implement those changes here.

I got yelled at by an ER doc on a rotation, because I would not agree to an involuntary hold. The patient was not an imminent harm to herself or others and was at her baseline.

How do you plan on changing it without going into some kind of policy making? Or owning of these facilities yourself? Genuine question as to how docs can have these goals achieved as individuals
