It is elicited by inflating a blood pressure cuff around the upper arm to a pressure higher than the patient's systolic blood pressure and maintaining it for 3 minutes. This occludes the brachial artery and causes ischemia in the distal nerves, which, if the patient has hypocalcemia, will result in carpal spasm. The hand and fingers will exhibit flexion at the wrist, metacarpophalangeal joints, and extension of the interphalangeal joints, resembling the position of a hand in the shape of a swan neck.
