Witcher scabbards ussually have coutouts from the side to allow the sword to be drawn. Its still difficult to sheath it but it isnt impossible.

If i remember right Geralts scabbards do have slits on the side to explain the sword and the guy from shadow of mordor has a cutout on the back of his scabbard and you can actually see it being used

Lord of the Rings: Shadow of Mordor/War are the only games I know of that do back mounted sword's in a plausible way. The top half of the scabbard is open so you only need to pull it out halfway.

my favorite back scabbard design will always be from shadow of mordor. there's an opening down a 3 of it so the blade can actually go in/out correctly
