The DESTINY-Breast 09 was presented at ASCO 2025 and showed that trastuzumab deruxtecan (Enhertu) plus pertuzumab (Perjeta) demonstrated a highly statistically significant and clinically meaningful improvement in progression-free survival (PFS) (time without progression or death) compared to taxane, trastuzumab and pertuzumab (THP) as a first-line treatment in patients with HER2 positive metastatic breast cancer. Median PFS was 40.7 months with Enhertu plus pertuzumab compared to 26.9 months for THP, reducing the risk of disease progression or death by 44%. There were more dose interruptions and dose delays w Enhertu and a 12% incidence of interstitial lung 🫁 disease, which is consistent with other studies of Enhertu. Approximately half of the patients in the Enhertu plus pertuzumab arm had de novo disease, meaning the breast cancer was first diagnosed in the metastatic setting. Some questions remain. Is this the new approach for everyone or just some patients? How do we manage the side effects with Enhertu ? Is there a role for Enhertu initially and then maintenance Herceptin/Perjeta and there is a study that will look at that. How do we bring in endocrine therapy and potentially the use of a CDK 4/6 inhibitor (based on PATINA study). Lots to consider!!
