Had a barbarian drink the mana potion I gave the party at the beginning of the campaign. For the entire boss fight, he had two level-2 spell slots. He did not know any spells.

I like to secretly roll my alchemist pots then hand out all same type usually one person gets the daily free rolled random one. I gave boldness out every one but r barb there's turned out be Alter self he got the temp HP atleast but dm ruled I got decide he altered in to bez dm liked idea was decided upon on Elixers creation... so r barbarian went from a orc to a minotaur... (night before he wanted know felt like be a horse and have horns)

I had some artefact with dragon blood in it that was supposed to give you its power for a short tile, when you broke it. I used it when a dragon attacked us and it turned out it was a cut scene 😭

Deny the reaper potion, full heal and double your max hp, +10 to all stats, crit on a 15+, 1/2 all damage, spell immunity for 7th lvl spells and below, and 2 extra attacks. You'll die in 3 rounds but enjoy your blaze of glory, make it count 🫡

Aaaand this has now inspired me to create a creature called a ‘Glask’. A silica-based creature who’s internal organs are stored within the cork of it’s glass bottle body. The liquid inside is its digestive fluid, which color varies on what contents it just digested/is digesting. (Not harmful to humans, even when drank. Some humanoid species will have mild to severe discomfort for 3d4 hours after drinking. Maybe 1d4 damage to creatures like plasmoids and animals who consume the whole bottle of liquid, and experience extreme pain.)

ok but our druid literally had this. she was dying, took a random potion she got from a hag hoping it would help. it was a love potion. first thing she saw afterwards was our party's goat (we have him as a pack mule pulling a cart)

I tried this move and it turned out to be a potion of water breathing. Absolutely useless in the fight, but it did mean my character got to sit inside of a barrel of wine for the remaining duration after we won.

I have my players a greater healing potion at level 3... it was actually a poison potion. The Barbarian had 5 hp left against the boss... the poison did 4d6 damage.

I did this against a Warlock enemy when all my party member were down except me. The potion turned out to be a potion of invulnerability defaulted to Necrotic attacks. I was immune to his spells for one turn which gave me enough time to set up for a whole party resurrection. We BARELY won, and still suffered heavy losses regardless. Tried another time, and it turned out to be a potion of shrinking. I was swooped away by a bird and never seen by my party again. Then again, it did lead me into another party group when it turned out that the bird was someone else's Kenku Artificer that made artificial wings. Now THAT campaign was a doozy.
