Considering Papyrus had never seen a human other than Frisk, there was no way he could tell an adult from a child. Sans's height could also be a factor, as Frisk and Sans's sprites are the same height (and Sans is obviously an adult), so Papyrus probably thought "Wowie, the human is small! Just like my brother!"
