And ultimately, the peer to peer is for determination of medicinal necessity. Not a guarantee of claims payment. A peer to peer can be done and a claim denial can still occur. On the bright side, insurance companies have taken a pledge to improve the PA system starting 2026

Universal healthcare does peer to peer in a more ethical way IMO, typically a broad policy is put in place for covered and uncovered procedures/meds/etc that is decided by a panel of physicians utilizing research to support their decisions. Then physicians can seek coverage for uncovered services, via an application & guidelines.
