Hello :
Emergency medicine has one job and it is to stabilize. That’s it. Once they’re stable enough, consultants can help determine inpt or outpt follow up. Psych is an entire field encompassing inpt, outpt, therapy, etc.

Easier in = easier out = more bodies to work. Alternatively though, EM is so broad and unpredictable that I’d imagine after 3 years you have the skills to find answers, not necessarily know everything

bc emergency doctors are actually needed and useful, so they have to be educated and able to staff an emergency department quickly. Psychiatrists are only pretend-doctors, they do absolutely nothing for 99% of their patients and their duties could easily be fulfilled by nurses, so they can be residents for longer, bc one psychiatrist is acutally enough for a big hospital

I work in the emergency department rn and the doctor I scribe for said that even after 4 years you still will not know everything and honestly probably will only know a fraction of the things you need to know and he said the most important thing you need to know is what to do when you don’t know something and I honestly thought that was a great piece of advice

A psychiatrist told me once psychology is a broad and sometimes quite vague speciality. Med school teaches clinical procedures, physiology, pharmacology and much more that deals with concrete evidence. I think med schools should focus on psych a bit more but youre essentially the "nurse" version of a doctor to a psychologist as well as a doctor. Idk enough about ED but 3yrs seems like its not enough.

As someone with little knowledge, my guess would be level of care. ER physicians are just first contact while a psychiatrist is there the whole time. Not sure if that necessarily good reasoning tho.

Because ER should be 4 years like it’s about to be. Way to many bedside procedures for it to be 3 years, too many programs are hitting bare minimum numbers from ACGME for said procedures to be graduate the residents.

It's not as 'hard"science as blood and guts. The knowledge doesn't necessarily build on past experiences.There are an infinite number of possible cases. And the 4 years residency doesn't necessarily help them much in working in a variety of jobs.

Because neurology is 4 years (psychiatry useful to be part of neurology. Freud, in fact, was a neurologist). I think that 4th year can GO but you’ll hear a variety of opinions on that from other attending psychiatrists.

By the end of my second year in EM I was nearly practicing independently. I saw sick patients from day 1 with attending only supervision. Our program did a ton of ICU time and we had no fellow or attending on at night so the senior in the ICU ran the unit. Was signed off on lines and had done tons of intubations intern year. Was signed off on ample amounts of procedures by the end. We did rare procedures days with cadaver and a lot of sim. We also rotated in the community EDs. You can absolutely learn what you need to in 3 years. But programs that are too academic have too many residents (including from other specialities) taking procedures and focusing om book learning rather than practical learning. And these hca programs do the opposite and don't care about resident education and just use them as work horses. I loved the middle ground of being a academic program that didn't have residencies for every single specialty
