When Tony Stark time-traveled to the past and met his father (Howard Stark), Howard noticed many of Tony's qualities in this mysterious man. This made him suspicious. He began to believe that his wife might have cheated on him—with Tony himself (since he doesn't know Tony is his future son). This misunderstanding deeply disturbed Howard. As a result, Howard became aggressive and emotionally distant toward his wife and future son (Tony). This traumatic childhood changed Tony drastically in that universe. He grew up feeling unloved and unwanted. Instead of becoming Iron Man, he turned bitter and vengeful. In that alternate universe, Tony Stark embraced darkness and became a villain — Doctor Doom.💀

if i'm not mistaken, in the original comics tony was not the biological son of howard stark. howard and his wife had difficulty having children. then an alien came to howard and made a deal so howard could have a child, but that child would be destined to pilot a destroyer in the future. after the son was born, howard hid him and adopted tony

So at some point before End Games Dr. Doom “Tony Stark” became aware or remembered who he actually was. In order to be able to resume his true identity Doom has to make sure Tony/Ironman dies. This makes sense with him dying at the end of End Games.

If you're implying the MCU doesn't have a Tony Stark, and that he and Viktor Von Doom are one in the same, I'm sorry but you're way off they're not about to retroactively ruin any Iron Man toy sales by saying he never existed. It's two separate characters.
