I feel like I became a better nurse the day I stopped being argumentative with patients. It's a waste of time. Sometimes you just have to let it play out for them to see the light. I'll do the risk vs benefits convo, but then we'll move forward and I'll be supportive

I always choose an NP over an MD if I can. I work in medical research at a College of Nursing. Nurses are trained to listen to patients in a way that MDs simply are not.

I mean, if it's doing apparently unnecessary testing to soothe a patient's concerns could be beneficial in theory/certain circumstances/etc. (leaving the cost issue to the side), I'd think there could be issues with creating lines/delays for some types of tests (MRIs, CTs, etc.) which could cause problems for those whose medical cases do indicate a need for the test.

I totally appreciate this but one really important point to consider is pretest and post test probability. If your pretest probability is low enough, a positive test doesn’t confirm the diagnosis.

The problem is that docs can’t just go around ordering everything a patient googles just for their psych well being. Docs get in trouble for that shit whole NPs & PAs are glorified for shotgun guess work.

I always consider my patients requests and preferences bc if they're part of the planning and agree to the course of treatment, they're more likely to adhere to it and get better outcomes. That being said, I DO NOT do unnecessary tests just to placate them. We discuss risks vs benefits, costs, changes in the plan based on the results from this test or not and make an educated, collaborative decision. I do have the final say because thats why they are here, for my knowledge and expertise. And I always tell them they are welcome to find another provider willing to do xyz. I have rarely had anyone actually leave to do that. My patients always appreciated that they are heard and I do have a really good track record for plan adherence and patient outcomes. Unnecessary testing though? Testing to make them happy? Sorry, that's not it

I agree with her… patients should have shared decision making.

Like there is truth here, but you could use this logic to give anyone antibiotics for no reason when they ask, no? “But I always feel better after I take antibiotics”
