@aero.total: I keep getting DM's asking if the fuel cutoff switch can be easily bumped from run to cutoff. They are trying to blame human error so that dreamliners, operating around the world, don't have to be grounded. Thus avoiding operations chaos and financial losses. Dead pilots can't defend themselves. There have been many instances in the past where they were trying to blame the pilots. However the pilots survived the crashed and defended their case successfully. Sully is a good example.

Easy to move accidentally = NO Easy to move deliberately = YES there should be a system in place where they cannot be moved at all during flight unless the aircraft detects certain faults like with the engines etc. or can be manually moved by unlocking them via a FMS command.

There was a service bulletin to check these switches and air India may not have done so. If the 1st officer is doing the take off he has his left hand on the throttles, above these switches, the captain has his right hand in a guarding position behind the throttles and just hovering above thes switches. If the plane is bouncing on the runway the back of his hand can strike them, if they’re worn then he could knock them off, the air India pilot turned them back on 10 seconds later and the port engine started to relight but had no thrust at that point. Boeing should have fitted these switches with “ missile switch” covers.

It does not matter how damn easy it is to move them, it should NOT be possible to switch off BOTH fuels while the plane is in flight. Once one is switched off the other should lock. So it is a design flaw no matter how the aircraft company tries to spin it.

People tend to forget this is air India we’re talking about here. There’s a good chance those safety latches got worn out over time and maintenance never bother to replace those switches

Yes, the switches have a locking mechanism designed to prevent accidental movement. But the official preliminary report states that the FDR recorded both switches being moved to CUTOFF at 08:08:43 and 08:08:44 UTC respectively. The same report also mentions a 2018 FAA bulletin (SAIB NM-18-33) warning of a possible defect in this locking mechanism that could allow unintended disengagement. The aircraft involved was not inspected following that advisory. So while the design includes a safety lock, even regulators acknowledged the risk of failure years ago

787 is latest technology. Questions ? How a super modern airplane can accept a switch off fuel to engine during operations (take off-cruise-landing) ? If not, data shows an order of cut, but not the physical move of the switch (coffee on the switch while taking off? Electronic problem)

no one is paying attention to the fact that a switch can be design with a solenoid and remote control that can look exactly like the oem. it is possible to replace the oem switch during servicing and no one will notice the difference. that is possible. THE POSSIBILITY OF TERRORISM SHOULD NOT BE RULED OUT.
