Hey, vet assistant here! I would recommend a cone until the hot spot is dry just because putting clothes on it traps the moisture and can cause spreading.

My sheltie recently developed a hot spot, and he actually ripped out his fur, trying to get to it! Vet has him on antibiotics, and the “cone of shame!” Also told us to put witch hazel on it, to help dry it up!

Change his food. If the first ingredient is corn it’s horrible for dogs. When I changed my Goldie’s food to lamb or rice based I didn’t have any more episodes of hot spots. What a relief!
