If she was contacted for a disciplinary council and they requested that she attend at a church owned location (where these all take place). If she attended would they just use that as an opportunity to say she violated the orders and then arrest her? This feels intentionally vague so they can always have the upper hand and also potentially trap her

Have you tried scanning it, editing out your name, spending the time to add all the names of ward members, family, any active member whose address you have, make sure to print envelopes with the church headquarters return address so they don't know it's from you. See if that helps.

Policy of the church is NOT inspired, but instead written by the church’s team of lawyers in order to protect itself from loss and liability. It’s a very well run business, protected by lawyers first and foremost and any actions that appear to be spiritual are secondary.
