@adampfau: Most people do skull crushers flat and straight to the forehead, but two simple tweaks make them way more effective. A slight incline puts your arms back just enough to increase the stretch on your triceps while taking pressure off your shoulders. Lowering the bar slightly behind your head also increases that stretch and takes stress off your elbows. You’ll feel the difference immediately.

I've been doing them with my head at the end so it's easier to get the bar lower. Not as much an incline though.
