Ex outpatient np here. The majority of the time the patients I would get who said that were actually taking it routinely and if you’re taking it like that, it does make your anxiety worse in the long run. I don’t mind prescribing it for a true panic disorder, but just having a panic attack doesn’t mean you have a panic disorder. These are supposed to be temporary and used PRN—the problem is they’re not.

My favorite psychiatrist I trained under always said.. there are risks on quality of life with long term benzo use… and there are risk of poorly treated anxiety… that’s where those crucial risk v benefit convos need to happen with patient and provider!

If it’s helping with panic why can’t a person take long term! Especially if they know what has worked!?!?

I have an autoimmune disorder/chronic pain for 10+ years, experienced abuse as a child, OCD, panic attacks, my current psych providers have talked a lot about PTSD, etc. I feel like more providers need to look at the bigger picture and the pt as a whole. Klonopin has kept me alive.

Literally 😭 my nervous system is shot and I’ve tried all of the meds they give me and they all make it way worse I asked for 0.25 klonapin I know it works it doesn’t leave me feeling bad the next day etc not even a daily dose just 15 pills a month for when I’m panicking they wouldn’t help me they’re “highly” addictive and okay fair but Zolofts fine tho even tho if can give you brain zaps if you wanna stop taking it also whose getting hooked on that low of a dose and not even having enough for daily use 😞

Bzp have a big problem. Protracted withdrawal syndrome. I am very reluctant to give these meds due to this phenomenom.

I wish more providers had your sense of understanding because it truly, and very literally, saves lives. they need to understand that sure, there may be a long term risk—but that long term is only even an option if your patient doesn't succumb to the acute untreated problem in the short term and self delete 😑

Im bipolar and was searching for meds that worked for yearssss. My current pmhnp (who is so awesome) prescribed me lithium and said its really common in the us for doctors not to prescribe lithium (and even discuss it!!!) despite it being so effective because they dont think bipolar patients will take care of their health/do bloodwork as needed with lithium. Paternalism in mental health is so out of control.

I used to ask for ONE pill of any benzo every 3 months just so I could attend the psychiatrist appointment 😭 I'd have to go hide in my car until it was time for the appointment and whole body trembling during the appointment, couldn't focus on anything lol

I’ve had so many providers act like taking 1 single Xanax pill a month for occasional severe panic attacks is addiction. I don’t get the big deal if taking sparingly and as prescribed? I also took a gene sight test and found Xanax was one of the only “green zone” meds for me!

It doesn’t solve the problem and it can actually mask it further and make it harder to deal with if you do eventually get off of it or become tolerant. I don’t personally see that much benefit outside of acute

I explain “let’s attempt to get this dose as low as we can. Ending up with about 5 a month for break case situation.” We titrate down and legit majority get to 5 a month with some left over each month ✌️

Can you give advice for someone who is getting major pushback from their new perscriber? He clearly sees my history and I’ve been on it but just refuses. I don’t want to doctor hop. He told me they cause Alzheimer’s and dementia…that’s his justification

So crazy! My psychiatrist has allowed me to take my lorazepam as needed even now when I’m pregnant. We talked about trying to avoid if possible 1st trimester which ended up being fine. But at 15 weeks when I had a full blown panic attack, a 1/2 dose is what calmed me down

They could always try vistaril. There’s alternatives to a medication that causes dependence and horrible and life threatening withdrawal symptoms.

It seems like Nps have more of a heart than psychiatrists. The previous one I was at after trying all the anxiety meds wanted to do seroquel and I was already on an antipsychotic.

Thank you. I know the DEA is down providers necks and then comes the opioid epidemic, which has left the chronic pain community and anyone else on controlled prescriptions as the innocent victims. It's all about Risk vs. benefit. Not to mention in my eyes, if the patient is going to abuse, you're going to find out pretty quickly. It's not long before they will lose their rx, drop it in their soup, run it over with the lawn mower because the bottle was in their pocket. I've heard it all. Do these freak things happen? Of course. When Mrs. Smith calls who's never had an incident in 15 years saying this happened, we're gonna believe her. When it's Mrs. Conner who constantly asks for early fills, can't pass a drug screen, and has a million excuses.... well... those are all red flags and grounds for terminating scheduled prescription therapy. I had an ER doctor berate me and tell me how dangerous my meds were, not having ever looked at my diagnosis. I don't know how I had the courage to say, "I understand there are risks with these meds, and you see people abuse them all the time. However, they can be life-saving when prescribed correctly and closely monitored, especially for conditions and diagnoses such as mine, which you haven't even looked at. I never once asked you for an RX while I was here; I'm not med-seeking, yet you glossed over everything the second you saw my med list." I reported him to the hospital when they followed up on my patient experience. It wasn't worthy of reporting him to the board, but I know he will remember me. The nurse apologized for him once he walked out, and said, "I'm so sorry, I understand, and I'm on your side." I've been on these meds for over 10 years. I'm seen every 90 days at minimum. I am subject to drug screens and I under a controlled prescription contract I sign every year. I uphold my side of responsibility. I fully understand the drs side and their license on the line as well. They need proper documentation, in person visits, proof you're not abusing, to physically see you show no signs of abuse etc. They'll check the state database every month before sending in your rx.
