I had the same issues. I went through the fistulagrams, I went and spoke with the dietitians OFTEN, I was doing all the right things, but I still had potassium issues constantly. My nephrologist associated it with my diabetes with no definite answer, none of it makes sense and the centers just make you feel like it’s your fault Even though you’re literally doing all the right things. This is a wicked disease and it’s so hard. Sorry that you’re dealing with this.

I’ve also been prescribed valtassa (different form of lokelma) once per day rather than only non dialysis days. Sometimes even twice a day.

I am pretty sure I've heard before something familiar, they can find the mechanism that's happening Hemodialysis → temporary potassium removal, but not sufficient.No food or insulin intake before and after → inhibition of potassium entry into cells. Catabolic state during sleep → potassium exits the cells.. Sleep / hypoventilation → possible acidosis → further potassium efflux. Waking up with hyperkalemia and symptoms (muscle weakness, cardiac arrhythmias, etc.).the doctor should find that mechanism instead of "your body is dying " everything fas a reason and mean something

I would be curious to see what your acid bath is. If they are using a 2k maybe switch to a 1k for the first 60min of treatment. Then back to 2k.

just a thought but how is your kt/v with your labs? is your fistula functioning properly? have you had it evaluated by your vascular surgeon recently? I've found in my practice that if your electrolytes are off despite dialyzing appropriately your access may not be functioning properly. I hope you can get this figured out.
