Underground utility work is often cut throat. The big wigs cut every possible cost to get low bid. safety measures are often the first to go. we don't cut corners so we are never the cheapest but they continue to reward companies that let this happen..

I was caved in on May 2. Broke my leg in four places, and my foot was twisted in 180°. I got lucky a guy was paying attention and yelled and I was able to dive just in time.
