Air leak obviously. Either around the carby and manifold, or cylinder bolts are loose and a broken cylinder gasket. Mostly cylinder. At level, gravity is holding down the cylinder down but when you point the bar down the cylinder slightly tilts forwards opening the air leak

Air leak on the carb boot more then likely, when the saw is tipped down it flexes the boot. Common issue when just jam a saw and just yank and rip on it to get it out

Crank seals, drawing air into the cylinder. Mix up some soapy water and spray around the crank seals and it will start bubbling 👍🏼

Either your anti vibrant stuff is a Bit loose and your saw gets on the Trigger or something or your Membran in the carburator is a Bit old and you should get a new one i have the Same Problem on my Stihl 08s but i dont change anything because i dont work with it anymore

I'm 100% positive that one of the bearing seals is done, and I tend to believe it's the clutch side who's damaged.

Have a look at your anti vibe mounts. If they are stuffed it'll pull on the throttle linkage as you tilt forward
