It’s because they’re not hashtags, they’re number signs. The idea is that you can reorder the problem list if you just use number signs instead of actual numbers. (I’m just a medical student but this what the IM people told me)

In CPRS (EMR at VA hospitals) they’re helpful for notes to differentiate sections bc you can’t bold anything. They also pull that condition into the problem list to choose from when you select diagnoses for the encounter
