but the transporter works by "disassembling your molecules here and then reassembling them there" but it's not about the matter moving from place to place. it works very similar to a fax machine. scans at 1 end, prints at the other, but there's no actual matter being sent, only data. this was all explained in the TNG episode when Tom Riker comes into existence.

one presumes that on earth at least, the transport network would be so ubiquitous as to not even require satellite, so your matter stream would go point to point till it reaches your destination, or it would be hard wired.

Well there are transporter relays and pattern enhancers as seen in several episodes but I would imagine that transporter beams can easily pass through solid matter using annular confinement beams. The transporter likely uses some form of high-energy electromagnetic or subspace-based radiation to carry the matter stream. This is pretty much explained in The Best of Both Worlds” (Season 3, Episode 26 & Season 4, Episode 1 The Borg cube’s shields block transport, and Data mentions the transporter’s “subspace field” failing to penetrate. This implies the beam’s some kind of subspace energy that shields can disrupt.

So, this was actually answered in Star Trek Magazine in the 90s. The matter stream can actually move as it needs to find the receiving target and is broadcast to the specific terminal in a number of ways, be it ground to air, air to ground or through conduit lines connecting remote spots on the same planet. Most often, though, it bounces the stream off of a number of additional targets in the atmosphere to reach the desired location.
Please also note that they do not experience any time passing as they are moving at the speed of light while being teleported.
