I wrote this last week but it’s ever more important. I’m sorry the system is not working for a lot of women. I say sorry about 14/15 times a day minimum, whether that’s to the people in my care, the people I interact with, staff members or my family because I’m always late home or not mentally present when I am home thinking about work. 13 years in & I don’t think there are many weeks where I haven’t questioned staying in the profession. I speak as a really passionate midwife who fundamentally loves my job, but where the system has been so underfunded despite so many reviews & so many recommendations never met by governments.  There are no national safe staffing levels for caseloads of community midwives, no safe staffing ratios for triage or assessment units or wards, no safe staffing for women unless they are ‘in active labour’ where 1:1 care is then recommended. Yet safe staffing is the fundamental basic to providing woman centred care, to providing continuity, to listening to women, to truly being with them and supporting them. In Australia & Canada there are safe staffing ratios in maternity services. The RCN has a safe staffing report yet the RCM do not. I long for the day where midwives can have a lunch break & leave on time, can complete their training in work hours & not in own time. Safe staffing has been on almost every report, CQC report, investigation into units for decades & yet it has fallen on deaf ears. It is midwives & maternity support workers who are failed with a high attrition rate AND most importantly the women & babies who ultimately suffer and deserve better.  You don’t stop physically or mentally as a midwife when you work clinically. You don’t stop emotionally when you get home. You are non stop on your feet trying your utmost to provide care. Trying to meet the safety, needs, expectations of women & people. What you want to give versus what you are able to are polar opposites. You take that guilt, those emotions, those reflections home with you wishing you could change the system. The dharzi report highlighted last year the average midwife has 22 days off a year sick with burn out. It is an emotionally draining job with absolute highs & lows & one which would be manageable if the workloads & ratios were better, where we were able to provide the level of care we strive to & want to provide, in the right culture with the right support in place & having the basics met.  I want to believe an investigation will change the system. But after 13 years of the same recommendations never having the actual funding or investment and maternity constantly told to do more with less resource & money nationally, with no safeguards and no safe staffing I am a tired midwife who has to remind myself daily that we are trying our best & maybe, just maybe the system will receive the funding it needs for all women across the country to have the birth & support they deserve & need. Because I’m not sure I can take another report with the same recommendations ignored. I don’t know how much of myself I have left to give. I don’t know how many more women I can hear about who have been let down across the country. Birth is not just one day.
