There has to be like some underlying trauma for him to be so attached to his teddy bear, like maybe since it was a gift and his parents passed that bear is the good memory to keep, idk that how I think

as a CSA victim and someone who THOUGHT he had a close relationship with a stuffed animal (he just stays in my pocket and I show him to friends or make jokes about him rarely..) this is simultaneously very strange and very concerning. a few things are surely at play.
the story of screaming when teddy was taken away, combined with his poor communication skills and lack of regulation/social awareness tell me that whoever raised him was either incredibly neglectful, or a helicopter parent. perhaps with other forms of abuse on top. it also tells me he has gone through an extreme form of trauma, and I do not say that lightly. his past is incredibly dark. I have been through shit that you cannot even imagine, and im not even nearly this dysfunctional. that being said, I could be closer to this if I hadn't found a support system at about 17. but still not on this level. I can imagine the things hes gone through are on the level of a horror movie. this is NOT the coping mechanism of someone whos had even a chance to adjust properly or even a couple years of support from people who wouldnt hurt him.
it is not her responsibility to help him, they've been together only a few months and she is unhappy. for her, I would honestly say she should just move on and find someone else for her own sake as this is clearly not the right environment for her. for him, I hope he can find support to heal from his past, or at least is able to be happy how he is without making others uncomfortable.
