@jonjon.jpeg: Yesterday there was a horrific crash. When I came back today, there was another. Both involved teenagers on bicycles. In each case, the fire department, police, and eyewitnesses will work together to assign blame. Who made the last mistake that could have prevented this? But that focuses on the wrong issue. It looks only at the active error, the final mistake. Real solutions come from looking upstream, at what are called latent errors. The goal is not to assign blame, but to reduce how often we ever need to. #carculture #crash #pedestriansafety #trafficsafety #urbanism

my car was totaled last year at an intersection where there is an almost identical accident every 2 weeks. been this way for 20 years. city WILL NOT put in a red light with turn signal there. its just a stop sign where the secondary meets the main road

A child riding a scooter was hit by a car in my town. The local news spent 5 minutes of their 30 minute show lecturing viewers on scooter safety as if *we* were all children. It was obnoxious to say the least. 🙄

I learned about this concept from the perspective of software engineering. It’s called “root cause analysis” and there’s a technique called “the 5 whys”. The 1st “why” gets you to the “active error” and then the other 4 “whys” get you to the latent errors further and further upstream. Whenever something goes wrong, like an outage for example, we have a “retrospective” meeting where everyone comes together to discuss the “root cause analysis” and then discusses what we can do differently in the future to ensure that, if this exact same series of events happened again in the future, we would not have another outage. It’s not about blame, it’s about turning a failure into a learning opportunity and making your systems stronger and more robust
