i’m a nurse and I respond to messages within 1 hr at the most. I don’t allow patients to get treated over mychart and will send them a link to schedule an appt unless pt discussed it with you at a prior visit. we do not provide TREATMENT over mychart it needs to stop

1. If quick question, can answer quickly. 2. If I have to spend more than 3 minutes in the chart or if I have no clue, come to clinic. 3. If the message is a whole paragraph, that's a clinic visit.

My neurologist answers usually same day. He has been incredible with med changes, my slight mental breakdowns, questions, etc. I know this is not typical and I am INCREDIBLY grateful (also careful not to abuse it).

I use it cause I know I’m speaking to the nurse. If it went directly to the dr I’d refuse to use it. If I’m not sure if I need to be seen I leave the message open like hey can I take care of this at home or do I need to schedule an appt.

I’m in Canada and I don’t think that’s as common here. I certainly don’t know what I’d use it for. Even for med refills I just get the pharmacist to fax over a request. Anything else I’d make an appointment 🤷♀

Another huge problem is that this time that clinicians are spending doing patient care is not reimbursed at all. Because it’s not reimbursed, I do not get time built into my schedule to answer messages. I’m supposed to do it at the end of my workday. If this was a billable service, I bet we would get more adequate time to address these things.

Physicians should be allowed to charge for messages , unless related to ongoing care clarification, simple and succinct request for referral. The messages have exploded and take time away from other patient care.

New problem? Appointment or triaged by RN to determine if needing ER. Follow up for prior problem- most of the time I’ll take care of in mychart unless it’s super complicated or there are a bunch of new things going on or we haven’t seen you in forever.

My docs great about it. If I’m following up with someone in another specialty and I’ve had my standard physical with my doctor, I feel like a message through mychart is fine. If I need a random lab added on or want to catch him up the recommendations from the other specialist and nothing has changed with my physical health, why would I come in or waste his time with a virtual? But yeah I always try to make my messages succinct. Also our docs are now allowed to charge per message if it takes them enough time and they’re giving medical advice that they deem worthy of a “visit” essentially.

To be fair your options for texting etc work better bc you are able to have a smaller patient list for the day. I’ve had good and bad experiences with my chart. I’m now spoiled by your office 🤣 the large clinics or offices need to have one or two people and that’s their job bc waiting on nurses or docs in between hundreds of appts is not feasible in my opinion

I never have an issue with it. That said, I use it for basic things like refills, asking if I can get a lab added on to labs. I don’t disclose new symptoms etc.

Lack of early childhood/adult education focused around basic health related symptoms and the typical physiologic response. Society as a whole is nearly helpless to most complexities in today's modern Era. Forget helping everyone and focus on making your practice effective and efficient so that those who are fortunate utilizing your services remain treated.

Do not work like you have billable hours. Every interaction with a chart or the patient is an actionable encounter and will be treated like one in a deposition. You’d be better off setting clear instructions on what is allowed in chats vs what is required for a visit assuring the patient can safely wait for an appointment. And if people don’t like this: blame lawyers, they boxed us into this mess.

Patients have unrealistic expectations from mychart 🫠🫠🫠, this is not a text messaging app. I don’t know what is the right answer, its difficult to fit everyone on my already overbooked schedule so I answer what I can, but also we get ZERO protected time or pay to deal with all this extra work 😩, and some things can’t be that simple to answer with a quick message, medicine is complicated…

My PCP always messages back within a few hours and I use it less because I know it’s her.. my baby’s peds office the nurses always get back to me and tbh I message all the time bc I know they won’t bother doc unless necessary

If I can answer in a short paragraph (3-4 sentences) I respond directly to the patient. If it’s something my nurses should know I message them and they tell the patient. If I (as the subject matter expert) can’t answer in a short paragraph, then it’s complicated enough we should talk about it in person.
