As long as you aren't leaning on the rails you're doing the same amount of work. I have to rest my hands and look at my feet to stay balanced on it and I still get a good workout!

No I get you! My fear of tripping and tumbling has me mostly watching my feet and then the workout feels like it goes by slower and it’s miserable but I will definitely fall off! (And for reference I am extremely balanced and do dance/gymnastics, stairmaster just is spooky to my inner ears)
