First guy sounds like kidney stones, absolutely excruciating but not life threatening. The other sounds like an infection, potentially UTI or even worse pyelonephritis. I'd see the woman first.

First = renal colic - pain killers and further Ix to see best Mx option , second = pylonephritis - can lead to sepsis will need A-E, sepsis 6, Abx - 2nd patient priority

woman 2 almost sounds like a UTI got out of hand and she is starting to go septic? I would see her first and order pain meds for the gentleman in the meantime. I'm not medical enough to know what else the woman could have but man sounds like kidney stones

I’m thinking the man has kidney stones which are excruciating but not life threatening and there isn’t a ton that can be done for them. It sounds like the woman has a severe UTI I’d see her first get cultures and start broad spectrum antibiotics, and then see the man give him necessary pain management

#2 first, possible pyelonephritis and most likely needs iv antibiotics. Then #1 for kidney stones, pain meds & CT scan🤪

#2 sound like pyelonephritis complicating a UTI and is developing sepsis (Drowsy, Flushed and unwell). she needs obs, antibiotics, a urine culture and likely fluid resus (+/- imaging if concerned for abscess complicating PN). Man has uncomplicated ureteric calculus. PR indocid +/- opioid protocol and CT kub to localise/characterise stone.
