i had a similar rash for years that always worsened with my MCAS. they sent me the rheum to rule out lupus, then to the derm who dxed roseacea and told me that the majority of malar rash is roseacea, but because it can be lupus they always rule that out first. i still get mild recurrence with bad MCAS flares, but it's almost completely controlled by azelaic acid now. if the allergist isn't helpful, my dermatologist has been way more helpful than I ever expected. hope you get some answers/solutions!

If western medicine can’t give ya an answer see an acupuncturist, mine fixed my swollen face when my GP gave up on me 😅
